Application for the re-issue of a matriculation certificate


Description

You are issued a matriculation (matric) certificate when you pass Grade 12.

A matric certificate may be re-issued to people whose matric certificates have been lost, stolen or damaged or to people who have changed their surnames.

Steps to follow

  • Apply in person at any national or provincial office of the Department of Education.
  • Complete an affidavit about your previous certificate at the Department of Education.
  • Provide details of where and when you wrote your examinations, including the following:
    • Year
    • Province
    • School or examination centre
    • Your examination number
  • Provide your identity document
