Description
You are issued a matriculation (matric) certificate when you
pass Grade 12.
A matric certificate may be re-issued to people whose matric
certificates have been lost, stolen or damaged or to people who
have changed their surnames.

Steps to follow
Apply in person at any national or provincial office of the
Department of Education. Complete an affidavit about your previous certificate at the
Department of Education. Provide details of where and when you wrote your examinations,
including the following: -
